    Email Id : Applicants should fill their email ID, as the certificate and acknowledgment will be sent on the registered email ID. आवेदकों को अपनी ईमेल आईडी भरनी चाहिए, क्योंकि प्रमाण पत्र और पावती पंजीकृत ईमेल आईडी पर भेजी जाएगी।

    Mobile Number : Applicants must fill his / her mobile number. do not add +91. आवेदकों को अपना मोबाइल नंबर भरना होगा। देश कोड +91 से बचें

    BUSSINESS OF OFFICE ADDRESS : Applicants have to fill his / her complete office address. आवेदकों को अपना पूरा कार्यालय का पता राज्य और पिनकोड के साथ भरना होगा।

    DISTRICT & STATE : Fill Correct District & State. सही जिला और राज्य भरें।

    PINCODE : Fill Correct PIN CODE. सही पिन कोड दर्ज करें।

    Social Category : Applicants have to select at least one social category among General, OBC, SC, ST. आवेदकों को सामान्य, ओबीसी, एससी, एसटी के बीच कम से कम एक सामाजिक श्रेणी का चयन करना होगा।

    Aadhaar Number : Applicants are required to fill his/ her 12 digit Aadhaar number issued by Unique Identification Authority of India (UIDAI). आवेदकों को भारतीय विशिष्ट पहचान प्राधिकरण (यूआईडीएआई) द्वारा जारी अपना 12 अंकों का आधार नंबर भरना आवश्यक है।

    Pan Card Number : Applicants have to fill his / her permanent account number(pan) card number. आवेदकों को अपना पैन कार्ड नंबर भरना होगा।

    Bank Account Number : The applicant has to fill his / her bank account number. आवेदक को अपना बैंक खाता नंबर भरना होगा ।

    IFSC Code : Applicant can fill his / her bank IFSC code. आवेदक अपना बैंक IFSC कोड भर सकता है।

    Date of Commencement of Business : Applicants have to fill the date of commencement of business as it will be printed on the certificate. आवेदकों को व्यवसाय शुरू करने की तारीख भरनी होगी क्योंकि यह प्रमाण पत्र पर छपी होगी।

    Type of Organization : Applicants have to select the type of organization, as it will get printed on the certificate. आवेदकों को संगठन के प्रकार का चयन करना होगा, क्योंकि यह प्रमाणपत्र पर मुद्रित होगा।

    Business Name : Applicant has to fill business names, as it will be printed on the certificate. आवेदक को व्यवसाय का नाम भरना होगा क्योंकि यह प्रमाणपत्र पर मुद्रित होगा।

    Main Business Activity of Enterprise : Applicants have to choose the main business activity from the given options. आवेदकों को मुख्य व्यावसायिक गतिविधि का चयन करना होगा।

    Additional Details About Business : The applicant can fill additional details about the business. (for example – manufacturing of paper, Car washing service provider) आवेदक व्यवसाय के बारे में अतिरिक्त विवरण भर सकता है। (उदाहरण के लिए – कागज का निर्माण, कार धोने का सेवा प्रदाता)

    Number of Employees : The applicant can fill the number of workers in his / her firm. आवेदक अपनी फर्म में श्रमिकों की संख्या भर सकता है।

    Investment in Plant & Machinery / Equipment : The applicant can fill the total investment made in a factory, machinery, tools, etc. to start his / her business. आवेदक अपना व्यवसाय शुरू करने के लिए एक कारखाने, मशीनरी, उपकरण आदि में किए गए कुल निवेश को भर सकता है।

    Upload Aadhaar Card : Applicant can attach scan copy of Aadhaar card front side (jpg,png file < 12MB)
